When visiting an in-network eye care professional, there is no need for a claim form. A member simply provides his or her full name and date of birth or ID number at the time of service, and the eye care professional will then verify eligibility and benefits.

However, if an optical voucher is required by the Fund to obtain services, members must contact their Fund office directly to request an authorized voucher prior to using the benefit.

If your group’s vision benefit includes an out-of-network option, a claim form for reimbursement is available on the member portal. Members seeking out-of-network benefits are responsible for full payment for services rendered at time of service.

Yes, GVS’ network includes providers located throughout the country, however, networks are customized and are set up by plan.  Please visit the GVS website or call 1-800-VISION1(1-800-847-4661) for a location near you.
